10 JULY 2018: DALIAN, CHINA

An Air China co-pilot who was smoking in the cockpit caused a flight from Hong Kong to Dalian to suddenly drop 6,000m after he accidentally switched off the air conditioning system in an attempt to prevent the vapour from his e-cigarette from circulating into the passenger cabin. The flight deck crew was immediately dismissed and an investigation was launched, which could lead to the pilots’ licences being revoked.
